To Register with the Practice

blood_pressure3Patients are eligible to apply to be registered with the Practice if over 16, and living in the practice area.  You will be asked to complete a registration form and a new patient questionnaire.  All patients over the age of 5 will be given an appointment for a new patient health check with the Practice Nurse.

Applications may be made on behalf of any child or any adult who is incapable of making such an application by, in the case of a child, either a parent, or in the absence of both parents, the guardian or other adult person who has care of the child, and in the case of an adult, by the primary carer of that person or a person authorised to act on the patients behalf.
